Nurse Practitioner, Spine, (Larkspur), Full-Time, Days

About the role

Under supervision and in collaboration with physician staff, the Advanced Practice Provider (APP) performs physical examinations, treats common episodic and chronic healthcare problems, and emphasizes health maintenance and disease prevention. This role is designed for nurse practitioners and physician assistants, focusing on patient care in the clinic setting, in-office procedures, and patient education to ensure high-quality healthcare services and excellent patient outcomes.

Responsibilities

• Assess patient status by obtaining health history through interviews, chart reviews, and physical examinations; evaluate presenting illness, risk factors, family history, psychosocial situation, and cultural factors.
• Order and perform appropriate laboratory diagnostic and screening tests; consult with physicians and other clinicians for evaluation of illness.
• Integrate data to determine diagnosis and therapeutic plan, including identification of health risks.
• Develop and implement treatment plans by prescribing medications and/or injections in compliance with medical practice guidelines and state laws; educate patients regarding health promotion and illness prevention.
• Prepare and update documentation in the EMR, including examination results, diagnoses, medications, and treatments.
• Participate in peer reviews, chart reviews, staff education, and clinical guideline development to ensure compliance with standards and regulations.
• Act as a consultant to the clinical team on specialty areas; promote patient advocacy and confidentiality.
• Collaborate with physicians to manage acute and long-term medical needs; provide continuity of care through triaging patient calls and emails.
• Perform all other duties as required or assigned.

Assessment

• Elicit and document a complete health history.
• Perform and document a complete physical examination.
• Order appropriate diagnostic tests.
• Assess patient’s perception of health status, personal strengths, support systems, and potential healthcare problems.
• Identify patients and families at risk for specific healthcare problems.

Diagnosis and Planning

• Formulate diagnoses based on health history, physical examination, and diagnostic tests.
• Develop and document healthcare plans with patients and families, including treatment, health maintenance education, and counseling.
• Establish, review, and document short and long-term goals in collaboration with patients.

Implementation

• Provide appropriate care based on individual needs, including age-specific considerations.
• Institute treatment plans based on assessments and diagnostic test interpretations.
• Counsel patients according to their motivation and compliance.
• Provide telephone advice and triage for acute and unscheduled patients.
• Document all pertinent data and interventions in patient records.
• Provide refills for routine medications.
• Take appropriate action when reviewing abnormal lab data and diagnostic tests.
• Consult with other providers as indicated.

Evaluation

• Evaluate patient understanding of and compliance with the healthcare plan through regular follow-up and reassessment.
• Re-evaluate short and long-term goals with patients and participating health professionals.

Education

• Patient and Family:
  • Identify learning needs of patients and family members.
  • Assess patient’s readiness to learn.
  • Provide individualized health education.
  • Develop teaching goals and objectives that meet patient and family healthcare needs.
• Staff:
  • Function as a clinical resource person.

Continuity and Coordination

• Function as the healthcare provider for a specified group of patients.
• Coordinate patient care activities and health care planning among multiple providers, patients, and their families.
• Communicate and consult with family, staff, and unit personnel prior to and during hospitalization; participate in discharge planning as appropriate.
• Promote ongoing communication with community agencies to facilitate continuity of patient care.

Advocacy

• Promote patient and family participation in planning their care.
• Assist patients in obtaining information necessary for informed decision-making and support their choices.
• Inform patients of the Patient Bill of Rights.
• Provide referrals to community resources when appropriate.

Requirements

• Education (Nurse Practitioner):
  • BSN required; MSN preferred.
  • Successful completion of advanced practitioner training and certification as a Nurse Practitioner from an approved program.
• License/Certifications:
  • Current Registered Nurse license from the California Board of Registered Nursing.
  • Active and unrestricted Nurse Practitioner license in California.
  • Current Nurse Practitioner Furnishing License and verification of experience to furnish (if applicable).
  • National board certification by the first day of work (must maintain certification).
  • Active and transferable DEA license to California as required by clinical specialty.
  • Eligibility for payer enrollment with CMS Medicare and Medicaid.
  • BLS Certification (American Heart Association) required.
• Experience:
  • 2 years of experience in advanced practice or specialty preferred.
  • 2 years in specialty area highly desired.
  • Prior experience in a medical office environment preferred.

Pay

The pay range for this role is $85.95 - $100.27 - $117.50 per hour, compliant with applicable law. Individual pay is determined by skills, experience, qualifications, and internal/market equity. Positions covered by collective bargaining agreements are governed by those agreements.

Schedule

This is a full-time position with scheduled weekly hours of 40, working 8-hour day shifts.
