About the role
We are seeking a Nurse Practitioner to deliver effective, equitable, and value-based primary care to adult Medicare patients.
Responsibilities
- Conduct office visits for routine and acute issues
- Perform wellness visits and health assessments
- Coordinate care with providers and specialists
- Provide patient education
- Manage care and conduct outreach
- Perform occasional home visits
- Work within a collaborative outpatient team that includes physicians, nurses, medical assistants, and support staff
Requirements
- Master of Science in Nursing or related certification
- Active licensure
- DEA registration
- Bilingual proficiency in Spanish

- Certification and licensure
- Language skills necessary for effective patient communication and care

- Familiarity with HCC documentation
