About the role
This role is for a Nurse Practitioner providing primary, acute, and preventive care to adult Medicare patients in an outpatient setting.
Responsibilities
- Conducting office visits
- Administering wellness exams
- Care coordination with other providers
- Patient education
- Participating in outreach and home visits as needed
- Collaborating with a multidisciplinary team, including physicians, nurses, and specialists, to deliver value-based, patient-centered healthcare
Qualifications
- Master's, DNP, or NP certification
- Active state license
- DEA license
- Bilingual proficiency in Spanish
- Preferred experience in primary care, geriatrics, or family medicine
- Knowledge of HCC, ICD-10 coding, and health assessments
- Team-oriented with a passion for innovative population health models
