This position is focused in a Skilled Nursing Facility (SNF) setting, where you would serve as a clinical leader, working collaboratively with the facility’s care team to oversee both short- and long-term residents.

Responsibilities

• Admissions, discharges, monthly stability checks, and evaluations for changes in condition.
• Advanced care planning and comprehensive assessments.
• Prescribing treatments.

Requirements

• Active Texas Advanced Practice Licensure (APN) and DEA License.
• Previous nursing or clinical experience.
• Strong communication and teamwork skills.
• Master’s degree in Nursing.
