Nurse Practitioner: Remote Telehealth Provider- Urgent Care

Responsibilities

  • Conduct telehealth evaluations for Medicare beneficiaries experiencing urgent, non-emergent clinical concerns.
  • Evaluate symptoms, review medical histories, and determine appropriate interventions.
  • Determine when escalation to the member's primary care provider, emergency department, or 911 is clinically indicated.
  • Document all encounters accurately and timely in the electronic medical record.
  • Avoidable emergency department visits and hospitalizations through proactive clinical intervention.
  • Provide patient education on disease management, medications, lifestyle modifications, and symptom monitoring.
  • Promote medication adherence and self-management of chronic conditions.
  • Provide clinical guidance and education to nursing staff on appropriate escalation pathways.
  • Participate in clinical case reviews and quality improvement initiatives.

Requirements

  • Active, unrestricted Nurse Practitioner license.
  • National certification appropriate to specialty.
  • Minimum of 3 years of clinical practice experience, preferably in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management.
  • Experience managing hypertension and common chronic conditions, including diabetes, heart failure, COPD, and chronic kidney disease.
  • Prior telehealth or virtual care experience.
  • Strong clinical assessment and decision-making skills.
  • Excellent verbal communication and patient education abilities.
  • Familiarity with eClinicalWorks (eCW) or similar EMRs.
