Responsibilities
- Conduct telehealth evaluations for Medicare beneficiaries experiencing urgent, non-emergent clinical concerns.
- Evaluate symptoms, review medical histories, and determine appropriate interventions.
- Determine when escalation to the member's primary care provider, emergency department, or 911 is clinically indicated.
- Document all encounters accurately and timely in the electronic medical record.
- Avoidable emergency department visits and hospitalizations through proactive clinical intervention.
- Provide patient education on disease management, medications, lifestyle modifications, and symptom monitoring.
- Promote medication adherence and self-management of chronic conditions.
- Provide clinical guidance and education to nursing staff on appropriate escalation pathways.
- Participate in clinical case reviews and quality improvement initiatives.
Requirements
- Active, unrestricted Nurse Practitioner license.
- National certification appropriate to specialty.
- Minimum of 3 years of clinical practice experience, preferably in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management.
- Experience managing hypertension and common chronic conditions, including diabetes, heart failure, COPD, and chronic kidney disease.
- Prior telehealth or virtual care experience.
- Strong clinical assessment and decision-making skills.
- Excellent verbal communication and patient education abilities.
- Familiarity with eClinicalWorks (eCW) or similar EMRs.
