About the role

We are seeking a Nurse Practitioner or Physician Assistant to work in collaboration and consultation with physicians, staff RNs, and other health care professionals to provide high-touch, compassionate care to patients and families. This role includes care coordination, care management activities, and practicing within the context of collaborative management with physicians to diagnose, manage, and prevent acute and chronic illness and disease while promoting wellness.

Responsibilities

  • Perform comprehensive adult/geriatric admission history and physical exams.
  • Identify, diagnose, and document episodic, acute, and chronic disease in adults.
  • Recognize conditions, characteristics, and behaviors that place seniors at risk.
  • Pursue feedback, continuing education, and professional development.
  • Effectively reconcile medications.
  • Identify and document the needs of clinically complex patients.
  • Work collaboratively with physicians, case managers, social workers, family members, caregivers, and ancillary medical personnel.
  • Participate in Quality Assurance Performance Improvement (QAPI) activities.
  • Assess the medical necessity and effectiveness of ancillary services; determine appropriate initiation of benefit events and communicate the process to providers and team members.
  • Coordinate care as members transition through different levels of care and settings, ensuring the Individualized Care Plan (ICP) is communicated within 24 hours to the receiving care setting.
  • Review and interpret medical and social history of members; interpret laboratory data and recommend appropriate follow-up.
  • Manage both medical and behavioral chronic and acute conditions in collaboration with a physician or specialty provider.
  • Synthesize information to diagnose and create a comprehensive plan of care.
  • Complete all regulatory required monthly, quarterly, and/or annual visits, as well as acute visits as needed.
  • Maintain appropriate and compliant documentation within the EMR (electronic medical record) software.
  • Ensure all diagnoses are ICD-10 coded accurately and documented to support the diagnosis at that visit.
  • Review orders and interventions for appropriateness and response to treatment to identify the most effective plan of care.
  • Participate in family conferences and provide advanced care planning and education when needed.
  • Address pharmacy recommendations or pharmacist communications regarding residents.
  • Be present in assigned facilities during work hours to ensure adequate face-to-face coverage for resident care and facility needs.
  • Practice ethically and in accordance with the Scope and Standards of Practice of their profession and Board Certification.
  • Fulfill rotating on-call responsibilities and after-hour coverage; travel between care sites is mandatory.
  • Perform other duties as assigned.

Requirements

  • Graduate of an accredited Adult or Family Nurse Practitioner Master’s Degree program or Physician Assistant Studies.
  • Minimum 2 years of clinical experience as a Nurse Practitioner or Physician Assistant in geriatric, adult, or family practice settings.
  • Proven ability to perform comprehensive adult/geriatric physical exams.

Qualifications

  • Current and unrestricted RN and NP licensure, or PA licensure, in the state of practice.
  • Board certification by AANP or ANCC in Adult, Family Practice and Geriatrics, or Adult-Gerontology Primary Care.
  • Current CPR certification.
  • Active DEA license.
  • Active national provider number (NPI) and active Medicare provider number.
  • Eligibility to participate in the Medicare program as a provider and registered.

Preferred Qualifications

  • Graduate of an accredited Geriatric, Adult, or Family Nurse Practitioner Master’s Degree program.
  • Previous long-term care experience strongly preferred.

Skills

  • Pertinent clinical work experience relevant to the population being served.
  • Ability to review and interpret medical and social history of members.
  • Interpret laboratory data, recommend appropriate follow-up, and reconcile medications.

Schedule

  • Monday–Friday with flexibility.
  • No evening or weekend onsite shifts required.
  • Rotating on-call responsibilities and after-hour coverage required.
