About the Role
In collaboration and consultation with physicians, staff RNs, and other health care professionals, provides holistic, compassionate care to patients and families, and delivers care coordination and care management activities on behalf of PruittHealth Physician’s Service. Practices within the context of collaborative management with physicians in diagnosing, managing, and preventing acute and chronic illness and disease, and promoting wellness. Nurse Practitioners perform advanced nursing practice that includes independent nursing functions based on nursing standards of care and a role in medical management under a collaborative agreement with physicians.
Responsibilities
- Perform comprehensive adult/geriatric physical exams
- Identify, diagnose, and document chronic disease in adults
- Understand, articulate, and incorporate palliative and end-of-life principles and practices into care plans and discussions with staff, patients, and decision-makers, including discussing, clarifying, and documenting prognosis, goals, and advance directives
- Recognize conditions, characteristics, and behaviors that place seniors at risk
- Pursue feedback, continuing education, and professional development
- Effectively reconcile medications
- Identify and document the needs of clinically complex patients
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel
- Participate in Quality Assurance Performance Improvement (QAPI) meetings as appropriate
- Assess the medical necessity and effectiveness of ancillary services to determine appropriate initiation of benefit events and communicate the process to providers and team members
- Facilitate agreement and implementation of the member's plan of care by engaging beneficiaries, facility staff, families, responsible parties, and primary/specialty care physicians
- Evaluate the effectiveness, necessity, and efficiency of the plan, making revisions as needed
- Utilize practice guidelines and protocols established by PruittHealth Physician’s Service
- Demonstrate adequate computer skills, including use of email, Microsoft Office Word & Excel, with effective written communication skills for reports and correspondence
Requirements
- Graduate of an accredited Adult or Family Nurse Practitioner Master’s Degree program
- Minimum 1 year of clinical experience as a Nurse Practitioner in geriatric, adult, or family practice setting
- Proven ability to perform comprehensive adult/geriatric physical exams
Qualifications
- Current and unrestricted RN and NP licensure in the state of practice
- Board certification by AANP or ANCC in Adult, Family Practice and Geriatrics or Adult-Gerontology Primary Care
- Current CPR certification
Preferred Qualifications
- Graduate of an accredited Geriatric, Adult, or Family Nurse Practitioner Master’s Degree program
- Previous long-term care experience
