Nurse Practitioner - PRN

About the role

Join a virtual healthcare team providing in-home health assessments to improve patient outcomes and close care gaps.

Responsibilities

• Conducting comprehensive adult and geriatric assessments, including medical history, diagnosis, treatment, and health education.
• Ordering and administering point-of-care and lab tests.
• Collaborating with physicians, case managers, social workers, family members, and caregivers.
• Providing services at members' residences, such as homes or Skilled Nursing Facilities.

Requirements

• Master’s Degree or equivalent experience with NP licensure.
• Current state licensure with prescription authority or ability to obtain it.
• Board certification in Adult, Geriatric, Family, Emergency, or Acute Care.
• Current BLS, ACLS, or CPR certification.
• Preferred one year of NP experience; new grads encouraged.
• Valid driver’s license and insurance coverage.
• Strong communication, computer skills, and familiarity with medical records software.
