About the role
Providing home-based primary care for medically complex, home-limited adults.
Responsibilities
- Perform in-home and telehealth visits to assess patient needs.
- Manage chronic and acute conditions through comprehensive evaluations.
- Coordinate care with an interdisciplinary team to ensure optimal patient outcomes.
- Order and interpret diagnostic tests and prescribe medications accordingly.
- Educate patients and caregivers to promote health and prevent hospitalizations.
- Document all clinical interactions in the EHR and support quality improvement initiatives.
- Contribute to expanding access to compassionate primary care for vulnerable populations.
Qualifications & Skills
- Current Nurse Practitioner licensure and relevant clinical experience.
- Strong assessment and clinical decision-making skills.
- Ability to manage multiple conditions in a home-based setting.
- Excellent communication and patient education abilities.
- Comfort with using telehealth platforms and electronic health records.
- Dedicated to providing patient-centered, compassionate care within a mission-driven environment.
