Nurse Practitioner Primary Care

Family Medicine Clinic – Join an established outpatient primary care practice serving patients across the lifespan. New graduates and experienced providers are encouraged to apply.

Schedule

• Monday–Friday, 8:00 AM–5:00 PM with potential to transition to a 4-day, 10-hour hybrid schedule
• No call or weekends
• Manageable patient volume: average 18 patients/day (8-hour schedule) or 24 patients/day (hybrid schedule)

About the role

• Comprehensive primary care for new and established patients, including follow-up visits, acute/same-day appointments, and chronic disease management
• Perform joint injections, trigger point injections, shave and punch biopsies, cryotherapy, and other simple in-office procedures
• Collaborative Enhanced Care Team (ECT) with physicians, APPs, medical assistants, LVNs, front desk staff, and additional clinical support
• Outpatient practice only – no hospital rounding or inpatient responsibilities

Responsibilities

• Assess the physical and mental condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories
• Order, collect, interpret, and evaluate laboratory and other diagnostic tests to assess patient problems and health care needs
• Prescribe, recommend, or use medical drug therapies or other treatments
• Perform procedures following approved protocols, focusing on efficacy, safety, and cost
• Recommend interventions to modify behavior associated with health risks
• Formulate and implement comprehensive patient treatment plans based on assessments, evidence-based medicine, and standards of care; collaborate with physicians and team members when appropriate
• Evaluate patient's response to care and its effectiveness
• Counsel patients and families on medical processes, illness management, medication, nutrition, and health promotion
• Counsel patients about drug regimens and possible side effects or interactions with food supplements, over-the-counter medications, and herbal remedies
• Maintain appropriate records detailing the patient's treatment plans and outcomes
• Initiate timely consultation and referral when the problem exceeds scope of practice or expertise
• Provide consultation level services for conditions/problems related to the provider’s specialty and training
• Perform invasive procedures specific to scope of practice and knowledge of the provider as delegated by supervising physician
• May provide care to patients in a team-based model, focusing on acute and chronic illness management and population health

Requirements

• Texas RN licensure or RN licensure with compact privilege from a Nurse Licensure Compact state for RNs and LVNs, and Texas APRN license
• Current license to practice as a Nurse Practitioner in the state of Texas
• Basic Life Support within 30 days of hire or transfer
• National Nurse Practitioner certification in area of specialty
• Graduate of an accredited program
• Less than 1 year of experience

• Knowledge of information and techniques to diagnose and treat human injuries, diseases, and deformities, including symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures
• Knowledge of practices, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions

• Skill in the use of computers and related software applications
• Ability to develop and implement comprehensive outcomes-based patient treatment plans
