About the role
This Nurse Practitioner Primary Care role provides compassionate, home-based primary care to medically complex, home-limited adults. You will work within a physician-led, interdisciplinary team to deliver comprehensive care through in-person visits and telehealth.
Responsibilities
- Perform comprehensive in-home assessments for medically complex patients
- Manage chronic and acute conditions in a home-based setting
- Coordinate care across physicians, nurses, and social workers
- Use telehealth to maintain continuity and access for home-limited patients
- Collaborate with the care team to improve health outcomes and reduce avoidable hospitalizations
Qualifications
- Valid Nurse Practitioner license
- Experience in primary care, geriatrics, or home health preferred
- Strong assessment and care management skills
- Ability to work effectively within an interdisciplinary team
