About the Role

Join an established Enhanced Care Team (ECT) model focused on collaborative, patient-centered care in an outpatient family medicine practice. This role involves:

• Caring for patients across the lifespan with a mix of new patient visits, established patient follow-ups, acute/same-day visits, and preventive care.
• Managing individual in-basket tasks with support from an experienced multidisciplinary team.
• Performing a variety of office-based procedures, including:
• Joint injections
• Suturing
• Incision & drainage (I&D)
• Skin lesion removal
• Skin biopsies
• Pap smears
• Nexplanon insertion/removal

Patient volume is approximately 18 patients per day. New graduates are encouraged to apply and will receive structured onboarding through in-clinic orientation, organization orientation, and the Thrive APP transition program.

Support & Resources

Work alongside an experienced care team, including:

• Medical Assistants
• Licensed Vocational Nurses (LVNs)
• Patient Service Specialists (Front Desk)
• Community Health Worker
• Additional APPs and Physicians

Schedule

Monday–Friday, 8:00 AM – 5:00 PM. No routine call or weekend requirements.

Responsibilities

• Assess the physical and mental condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories.
• Order, collect, interpret, and evaluate laboratory and other diagnostic tests to assess patient problems and health care needs.
• Prescribe, recommend, or use medical drug therapies or other treatments.
• Perform procedures following approved protocols, focusing on efficacy, safety, and cost.
• Recommend interventions to modify behavior associated with health risks.
• Formulate and implement comprehensive patient treatment plans based on assessments, evidence-based medicine, and standards of care. Collaborate with physicians and team members when appropriate.
• Evaluate patient's response to care and its effectiveness.
• Counsel patients and families on medical processes, illness management, medication, nutrition, and health promotion.
• Counsel patients about drug regimens and possible side effects or interactions with food, supplements, over-the-counter medications, and herbal remedies.
• Maintain appropriate records detailing the patient's treatment plans and outcomes.
• Initiate timely consultation and referral when the problem exceeds scope of practice or expertise.
• Provide consultation-level services for conditions/problems related to the provider’s specialty and training.
• Perform invasive procedures specific to scope of practice and knowledge of the provider as delegated by supervising physician.
• May provide care to patients in a team-based model, focusing on acute and chronic illness management and population health.

Requirements

• Texas RN licensure or RN licensure with compact privilege from a Nurse Licensure Compact state for RNs and LVNs.
• Current license to practice as a Nurse Practitioner in the state of Texas.
• Basic Life Support (BLS) certification within 30 days of hire or transfer.
• National Nurse Practitioner certification in area of specialty.
• Graduate of an accredited Nurse Practitioner program.
• Less than 1 year of experience is acceptable; new graduates are encouraged to apply.

• Knowledge of information and techniques to diagnose and treat human injuries, diseases, and deformities, including symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
• Knowledge of practices, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions.
