Job Summary

The Clinician in a Post-Acute Care setting provides comprehensive evaluation, treatment, and care management services to patients transitioning from acute hospitalization to rehabilitation, skilled nursing, long-term care, or home-based care environments. This role focuses on improving functional outcomes, managing chronic and complex medical conditions, reducing hospital readmissions, and supporting safe transitions of care. The clinician collaborates with interdisciplinary teams, including physicians, nurses, therapists, case managers, and caregivers, to develop and implement individualized care plans that promote recovery, independence, and quality of life. Key responsibilities include conducting patient assessments, monitoring clinical progress, coordinating care across settings, documenting treatment plans and outcomes, educating patients and families, and ensuring compliance with regulatory, quality, and safety standards. The clinician serves as an advocate for patients while contributing to organizational goals related to clinical excellence, patient satisfaction, and value-based care initiatives.

Dept. Scope of Service

The BMH-owned or managed physician practices provide medical services in various specialties and offices including pediatrics, OB/GYN, cardiology, orthopedics, general surgery, family practice and internal medicine. The practices are staffed by physicians and other licensed providers who are supported by clinical staff (LPNs or medical assistants) and clerical staff (receptionists, transcriptionists, file clerks). Hours vary by practice.

Requirements

  • Education: Master's degree from an accredited adult nurse practitioner program.

  • Experience: Two years of nursing experience in internal medicine preferred.

  • Licensure/Certification: Current licensure as an APRN by the State of Vermont. Certifications as an adult or family nurse practitioner by a national organization, which is recognized by the Vermont State Board of Nursing preferred. Qualified to be credentialed by the BMH Medical Staff. Healthcare Provider CPR certification. DEA license.

  • Mental Efforts: Requires eye/hand coordination and manual dexterity. Normal and corrected vision/hearing. Potential exposure to blood/body fluids and communicable diseases. Helping and assisting with the lifting and/or moving of patients.

  • Physical Demands: Requires some exposure to communicable diseases or body fluids. Potential for CTD and multiple and diverse interruptions.
