Nurse Practitioner/Physician Associate

About the role

AgeRight Clinical Services (ACS) is transforming health care delivery and improving health outcomes for seniors in various settings including Skilled Nursing, Assisted Living, and Memory Care. This role is ideal for an experienced Family, Adult, or Geriatric Nurse Practitioner or Physician Associate who wants to work in a facility-based environment, enjoy autonomy, and utilize their clinical skills.

Responsibilities

• Collaborate with a multidisciplinary team including physicians, therapists, dietitians, and social workers to provide comprehensive care to patients.
• Develop and implement treatment plans based on patient needs and medical guidelines.
• Conduct physical exams, order and interpret diagnostic tests, and prescribe medications as appropriate.
• Provide education and support to patients and families regarding their health conditions and treatment options.
• Participate in ongoing professional development through continuing medical education (CME).

Qualifications

• Current, unrestricted Oregon state license, or ability to obtain.
• Experience in post-acute care or long-term care settings preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Passion for providing high-quality, compassionate care to seniors.

Schedule
