About the role

Astrana Health is seeking a skilled and compassionate Primary Care NP/PA to join our growing outpatient clinic in Anaheim, CA with Gateway Medical Center. This role is ideal for a provider who is passionate about delivering high-quality, patient-centered care with a strong focus on preventive medicine, chronic disease management, and value-based care.

Responsibilities

  • Provide and manage comprehensive primary care services, including physical examinations, evaluations, assessments, diagnoses, counseling, and treatment
  • Preventive care visits, wellness exams, and age-appropriate screenings
  • Manage chronic conditions such as diabetes, hypertension, COPD, heart disease, and other common adult medical conditions
  • Prescribe medications and treatment regimens in accordance with clinical guidelines and best practices
  • Develop individualized, evidence-based care plans in collaboration with patients and families
  • Maintain a continuity panel of patients under supervising physician with appropriate follow-up and long-term care planning
  • Order, interpret, and follow up on diagnostic tests, labs, and imaging
  • Coordinate care with specialists, behavioral health providers, and ancillary services
  • Direct and collaborate with nursing and support staff to ensure efficient patient flow and high-quality care
  • Participate in quality improvement initiatives, including HCC, 5-Star, and other incentive-based programs
  • Advocate for patients within the healthcare system to support cost-effective, coordinated care
  • Recommend workflow and operational improvements to enhance clinical efficiency, documentation, and patient experience
  • Uphold the highest standards of medical ethics, professionalism, and patient confidentiality
  • Other duties as assigned

Qualifications

  • Active and unrestricted NP/PA license in CA
  • Fluent Spanish
  • Active DEA registration
  • Current BLS and ACLS certifications
  • At least 1 year of outpatient primary care experience
  • Geriatric care experience
  • Experience in value-based care, community health, or managed care environments
  • Strong communication, teamwork, and clinical judgment skills
  • Passion for serving diverse and underserved populations
