Nurse Practitioner/Physician Assistant - Post-Acute Care & SNF (Full-Time)

Responsibilities
- Provide high-quality, patient‑centered care to residents in skilled nursing facilities.
- Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
- Communicate with facility nursing staff via phone or secure messaging as needed.
- Maintain reliable on‑site attendance for scheduled shifts.
- Ensure compliance with clinical standards, facility policies, and regulatory requirements.
Qualifications
- Active U.S. license and board certification by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
- Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine (among others).

Pay
- Estimated compensation range: $42,000 to $118,000 per year, based on expected earnings under an uncapped productivity‑based compensation model.
- Up to $50,000 in longevity bonus opportunities.
- Annual performance bonus opportunities tied to clinical excellence, exceptional patient care, and meaningful contributions to practice success.
Schedule
- Six‑hour facility shifts for full‑time positions; rounds generally start between 7 a.m. and 10 a.m.
- Freedom from on‑call and overnight requirements.
- Timely charting using a company‑provided iPad.
