Join a close-knit, mission-driven care community in South Haven, MI as a full-time Advanced Practice Provider serving local skilled nursing facilities. Responsibilities

  • Provide high-quality, patient-centered care to residents in skilled nursing facilities.
  • Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
  • Communicate with facility nursing staff via phone or secure messaging as needed.
  • Maintain reliable on-site attendance for scheduled shifts.
  • Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Requirements

  • Active U.S. license and board certification by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
  • Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
  • Ability to perform the essential functions of the position satisfactorily, with or without a reasonable accommodation.

Schedule

  • 6-hour facility shifts for full-time positions (rounds generally start between 7-10 a.m.).
  • No on-call or overnight requirements.
