About the role

The Advanced Practice Provider (APP) will support the Centers for Medicare & Medicaid Services (CMS) Transforming Episode Accountability Model (TEAM), an episode-based care model designed to enhance quality, coordination, and patient experience while reducing unnecessary utilization and readmissions.

Responsibilities

  • Guides patients through the full surgical journey, from pre-operative assessment and optimization through recovery and post-discharge follow-up.
  • Collaborates with physicians, care coordinators, therapists, and other clinical partners to provide both virtual and in-person care.
  • Identifies and mitigates clinical risks, addresses quality care gaps, and supports safe, successful transitions across care settings.
  • Combines direct patient care with clinical leadership responsibilities, primarily supporting geriatric patients undergoing orthopedic, cardiac, and major bowel surgery.
  • Manages a panel of medium- and high-risk patients and serves as a clinical resource for the broader TEAM program.
  • Helps refine care pathways, support program development, mentor colleagues, and advance evidence-based practices that improve patient outcomes, care coordination, and continuity across the Mass General Brigham system.

Requirements

  • Master's Degree in Nursing required.
  • Advanced Practice Registered Nurse [APRN-NP - State License] required.
  • 2 years of licensed nurse practitioner experience required.
  • Experience in Primary Care, inpatient Hospital Medicine, Geriatrics, Perioperative Medicine, or pre-operative optimization strongly preferred.

Qualifications

  • Education: Master's Degree Nursing required.
  • Licenses and Credentials: Advanced Practice Registered Nurse [APRN-NP - State License] - Generic - HR Only required.

  • Proficiency in electronic health records (EHR).

Pay

$126,838.40 - $173,867.20/Annual

Schedule

Hybrid work schedule with 40 weekly scheduled hours.
