Position Summary

Coordinates and delivers primary care

  • Working within their approved scope of practice.
  • Obtaining complete medical history and physical data on patients.
  • Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed.
  • Performing a complete physical exam and recording findings.
  • Collaborating with physicians in managing the medical needs of patients.
  • Ordering appropriate laboratory and diagnostic procedures.
  • Educating and advising patients regarding health and illness prevention.
  • Recommending community resources to meet patient and family needs.
  • Supporting patient access and care coordination through triage, follow-up, and management of urgent situations.

Supports BMG Programs

  • Participating in health education.
  • Maintaining related records and statistical data and preparing statistical reports as needed.
  • Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate vendors.
  • Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.

Organizational Responsibilities

  • Attends and participates in department meetings and is accountable for all information shared.
  • Completes mandatory education, annual competencies and department specific education within established timeframes.
  • Completes annual employee health requirements within established timeframes.
  • Maintains license/certification, registration in good standing throughout the fiscal year.
  • Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
  • Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self.
  • Adheres to regulatory agency requirements, survey process and compliance.
  • Complies with established organization and department policies.
  • Available to work overtime in addition to working additional or other shifts and schedules when required.

Education/Licensure/Certification

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program.
  • Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date.
  • Verified by local Credentialing Office.
  • BLS obtained within 1 Month (30 days) of hire date or job transfer date if required.
  • American Heart Association or American Red Cross accepted.
  • Current BLS and ACLS certifications if required.
