Position Summary

Coordinates and delivers primary care
- Working within their approved scope of practice.
- Obtaining complete medical history and physical data on patients.
- Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed.
- Performing a complete physical exam and recording findings.
- Collaborating with physicians in managing the medical needs of patients.
- Ordering appropriate laboratory and diagnostic procedures.
- Educating and advising patients regarding health and illness prevention.
- Recommending community resources to meet patient and family needs.
- Supporting patient access and care coordination through triage, follow-up, and management of urgent situations.
Supports BMG Programs
- Participating in health education.
- Maintaining related records and statistical data and preparing statistical reports as needed.
- Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate vendors.
- Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.
Organizational Responsibilities
- Attends and participates in department meetings and is accountable for all information shared.
- Completes mandatory education, annual competencies and department specific education within established timeframes.
- Completes annual employee health requirements within established timeframes.
- Maintains license/certification, registration in good standing throughout the fiscal year.
- Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
- Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self.
- Adheres to regulatory agency requirements, survey process and compliance.
- Complies with established organization and department policies.
- Available to work overtime in addition to working additional or other shifts and schedules when required.
Education/Licensure/Certification
- Graduate of an accredited Physician Assistant or Nurse Practitioner program.
- Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date.
- Verified by local Credentialing Office.
- BLS obtained within 1 Month (30 days) of hire date or job transfer date if required.
- American Heart Association or American Red Cross accepted.
- Current BLS and ACLS certifications if required.
