Position Summary

Coordinates and delivers primary care
- Working within approved scope of practice
- Obtaining complete medical history and physical data on patients
- Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed
- Performing a complete physical exam and recording findings
- Collaborating with physicians in managing the medical needs of patients
- Ordering appropriate laboratory and diagnostic procedures
- Educating and advising patients regarding health and illness prevention
- Recommending community resources to meet patient and family needs
- Supports patient access and care coordination through triage, follow-up, and management of urgent situations
Supports BMG Programs
- Participates in health education
- Maintains related records and statistical data and prepares statistical reports as needed
- Participates in interdisciplinary care conferences and assists with referrals and dissemination of information to appropriate vendors
- Assists in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes
Organizational Responsibilities
- Attends and participates in department meetings and is accountable for all information shared
- Completes mandatory education, annual competencies and department specific education within established timeframes
- Completes annual employee health requirements within established timeframes
- Maintains license/certification, registration in good standing throughout the fiscal year
- Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department
- Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self
- Adheres to regulatory agency requirements, survey process and compliance
- Complies with established organization and department policies
- Available to work overtime in addition to working additional or other shifts and schedules when required
Education/Licensure/Certification
- Credential(s): Graduate of an accredited Physician Assistant or Nurse Practitioner masters program
- Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date
- Verified by local Credentialing Office
- BLS obtained within 1 Month (30 days) of hire date or job transfer date if required
- American Heart Association or American Red Cross accepted
- Current BLS and ACLS certifications if required
