Nurse Practitioner / Physician Assistant - Inpatient Palliative Care

About the Role

Presbyterian Medical Group seeks a Board Eligible / Board Certified Palliative Care Nurse Practitioner or Physician Assistant to join our established practice in Albuquerque, New Mexico. This role serves as a consultant in Palliative and Hospice Medicine, primarily in inpatient settings with some hospice care responsibilities.

The role involves approximately 7-8 consults per day, focusing on expert symptom management, collaboration with care teams, and supporting patients and families through interdisciplinary care.

Responsibilities

• Collaborate with colleagues in managing patients in outpatient settings.
• Provide care to patients with critical and life-limiting illnesses, aligning medical treatment with patient goals.
• Deliver expert symptom management to maximize patients' quality of life.
• Build effective relationships with referring physicians and care teams to improve collaboration.
• Work with an interdisciplinary team in both Palliative and Hospice settings to support patients and families.
• Assist the Hospice Medical Director with documentation for hospice benefit certifications.
• Contribute to improving the quality of care for patients.

Requirements

• Nurse Practitioner:
  • Master’s degree in Nursing.
  • New Mexico Nurse Practitioner license.
  • Two (2) years of experience as a Nurse Practitioner.
  • AANC or AANP certification.
• Physician Assistant:
  • Certified Physician Assistant.
  • Two (2) years of experience as a Certified Physician Assistant.
  • Licensed by the New Mexico Board of Medical Examiners.
