About the role

Duffy Health Center, a non-profit located in Hyannis, MA, provides compassionate and skilled integrated healthcare for persons with complex health and social needs on Cape Cod. We envision a Cape Cod where all persons have access to quality health care, safe and stable housing, and lives filled with connection and purpose. Duffy Health Center is a Federally Designated Healthcare for the Homeless Program. This requires that most of the patients we serve are experiencing or have recently experienced homelessness or some other form of housing insecurity. Our patients have complex needs, and we offer wrap around care including Behavioral Health and SUD programs. Staff choose to work here because they believe deeply in the mission, and some staff have relevant lived experience themselves.

Responsibilities

  • Provides professional medical care for Duffy Health Center patients at Duffy Health Center facility and outreach sites or telehealth at the direction of the Chief Medical Officer.
  • Consistent with the delineation of privileges, work includes examining patients, conducting systematic and comprehensive physical assessments, developing and monitoring plan of care for patients, and working with members of the integrated team of professionals at Duffy Health Center to provide for the overall well-being of patients.
  • Promotes patient health through education and primary prevention, provides patient education regarding medications, treatments, anticipated outcomes, and referrals.
  • Makes referrals to specialists as needed.
  • Works effectively with members of the Duffy Health Center team of professionals, as well as with outside agencies and organizations.
  • Responsible for after hours coverage as part of Duffy Health Center Medical call rotation.
  • Conducts research pertinent to patient care.
  • Maintains current licensure and certification for the position and participates in continuing medical education to stay abreast of practices and trends.

Requirements

  • A candidate for this position must have a master’s degree in Nursing, with 3 to 5 years’ experience in nursing as an RN.
  • Must be licensed as a Nurse Practitioner in the state of MA, and successfully complete the credentialing process; must maintain current DPH, DEA, NP, CPR, and CCH privileges.
