Nurse Practitioner (Per Diem)
Job Summary
The field position will cover Manhattan and Bronx regions. MJHS is a large not-for-profit health system in the Greater New York area offering a range of health services including home care, hospice and palliative care, rehabilitation and nursing care, and the MJHS Institute for Innovation and Palliative Care.
About the Role
As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to an assigned at-risk panel of Elderplan members who reside in the community. The NP will ensure the development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as a clinical resource to the member, family, and interdisciplinary team.
Responsibilities
- Provide and/or coordinate primary clinical care services for assigned high-risk members and facilitate supportive care during transitions from acute or sub-acute care settings.
- Act as a team player, member’s advocate, and a clinical lead while working closely with the Interdisciplinary Team to coordinate member’s care and optimize health outcomes.
- Act as a resource and contact for member/designated representative regarding all aspects of member’s individualized care plan and disease process.
Requirements
- Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required.
- One to two years clinical experience in role of nurse practitioner required.
- Minimum one year in geriatric specialty.
- Previous clinical RN experience in acute, subacute and/or long-term care setting preferred.
- Prior experience in managed care a plus.
- NYS NP License, Registration, DEA certificates.
Qualifications
- Driver license and car required.
Skills
Master’s Degree in Nursing, experience in geriatric specialty, previous clinical RN experience, and NYS NP License are required skills for this role.
Benefits
Per diem rates for telehealth visits are $150, and in-person visits are compensated between $150-$210 (with $180 being an average FFS).
Pay
Orientation hourly rate: $90/Hour

Telehealth visits for community are compensated at $150, and in-person visits are compensated between $150-$210 (with $180 being an average FFS).
