Nurse Practitioner - Part-Time

About the Role

We are seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care, internal medicine, or geriatrics. This role involves providing personalized, coordinated healthcare for residents in long-term care settings, such as nursing homes or assisted living communities enrolled in Medicare Advantage plans.

Responsibilities

• Assessing patients' medical and mental health needs
• Developing and implementing care plans
• Prescribing medications and ordering diagnostics
• Monitoring treatment responses and effectiveness
• Collaborating with a multidisciplinary care team
• Exercising independent judgment in clinical decision-making
• Ensuring compliance with healthcare regulations
• Supervising and training staff as needed

Requirements

• Valid APRN or PA license
• Degree from an accredited nurse practitioner or physician assistant program
• Minimum of three years of clinical experience in geriatric populations
• Proficiency in Electronic Health Records (EHR) and Microsoft Office
