Nurse Practitioner - Part-Time

About the Role

We are seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine, preferably with a geriatrics focus. This role involves providing personalized, coordinated healthcare to residents in long-term care settings, such as nursing homes or assisted living communities enrolled in Medicare Advantage plans.

Responsibilities

• Assess medical and mental health needs of residents
• Develop and implement individualized care plans
• Prescribe medications and order diagnostic tests
• Monitor treatment responses and adjust plans as needed
• Collaborate with a multidisciplinary team of healthcare professionals
• Exercise independent judgment in clinical decision-making
• Ensure compliance with all regulatory requirements
• Provide training and supervision to staff as needed

Requirements

• Valid APRN or PA license
• Degree from an accredited nurse practitioner or physician assistant program
• Minimum of three years of experience in geriatric long-term care
• Proficiency in electronic health records (EHR) and Microsoft applications
