About the Role

Hawai‘i Pacific Health is seeking a motivated and driven Nurse Practitioner to join our Palliative Medicine team on the island of O‘ahu. The role focuses on providing compassionate, patient-centered palliative care, including symptom relief, pain management, and support for patients and families facing serious illness.

Responsibilities

Develop individualized care plans by assessing and managing complex medical conditions, addressing physical, emotional, and psychosocial needs.
Facilitate advance care planning for patients and families.
Collaborate in interdisciplinary team-based care.
Participate in quality improvement initiatives to enhance palliative care delivery and patient experience.

Qualifications

Hawaii Registered Nurse License
Hawaii Advanced Practice Registered Nurse License (APRN)
Current American Heart Association Health Care Provider Basic Life Support (BLS)
