About the role

Palliative Care Nurse providing direct patient and family care through consultations for patients with serious illness and limited prognosis who are not yet eligible for hospice care.

Responsibilities

  • Apply evidence-based palliative medicine practices ethically and effectively, addressing physical, emotional, spiritual, psycho/social, ethical, educational, and safety needs across patient age groups.
  • Utilize clinical expertise to develop personalized care plans based on comprehensive assessments.
  • Gather and interpret pertinent information from medical records, family, caregivers, and perform thorough physical, emotional, psychosocial, and spiritual assessments.
  • Complete assessments, interpret findings, conduct medication reconciliation, formulate diagnoses, initiate therapeutic plans, and evaluate outcomes.

Qualifications

  • RN/APRN licensure with prescriptive authority in the practice state.
  • Minimum 5 years of clinical nursing experience.
  • Current CPR and BCLS certifications.
  • Encouraged ACHPN certification after 2 years or upon meeting experience requirements.

Preferred Skills

  • At least 1 year of experience as an APRN in Hospice, Palliative Care, or Oncology.
  • Advanced practice palliative care certification (ACHPN).

Compensation & Benefits

  • Salary ranges from $95,686 to $143,530 annually, based on experience and qualifications.
