ESSENTIAL JOB FUNCTIONS
- Conduct comprehensive patient assessments, focusing on physical, psychosocial, and spiritual needs
- Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team
- Facilitate advanced care planning and goals-of-care discussions with patients, families, and healthcare providers
- Prescribe medications, treatments, and interventions consistent with licensure and scope of practice
- Provide clinical leadership and mentoring to the interdisciplinary team
- Ensure compliance with Joint Commission, state, and federal regulations
- Participate in quality improvement initiatives and program evaluation
- Travel required to patient locations throughout the New York metro area
- Works within legal, ethical practice standards to his/her position as well as Calvary's Code of Conduct
- Comply with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer
Requirements
- Current NYS Registered Nurse license
- Current NYS Nurse Practitioners license
- Master's or Doctoral degree in Nursing; certification as an Advanced Practice Nurse in Palliative Care or related specialty preferred
- Minimum of two years of clinical experience as a Nurse Practitioner in palliative care, hospice, oncology, geriatrics, or related field preferred
- New York State Driver's License preferred
- Demonstrated expertise in complex symptom management, advance care planning, and interdisciplinary collaboration
- Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions
