ESSENTIAL JOB FUNCTIONS

  • Conduct comprehensive patient assessments, focusing on physical, psychosocial, and spiritual needs
  • Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team
  • Facilitate advanced care planning and goals-of-care discussions with patients, families, and healthcare providers
  • Prescribe medications, treatments, and interventions consistent with licensure and scope of practice
  • Provide clinical leadership and mentoring to the interdisciplinary team
  • Ensure compliance with Joint Commission, state, and federal regulations
  • Participate in quality improvement initiatives and program evaluation
  • Travel required to patient locations throughout the New York metro area
  • Works within legal, ethical practice standards to his/her position as well as Calvary's Code of Conduct
  • Comply with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer

Requirements

  • Current NYS Registered Nurse license
  • Current NYS Nurse Practitioners license
  • Master's or Doctoral degree in Nursing; certification as an Advanced Practice Nurse in Palliative Care or related specialty preferred
  • Minimum of two years of clinical experience as a Nurse Practitioner in palliative care, hospice, oncology, geriatrics, or related field preferred
  • New York State Driver's License preferred
  • Demonstrated expertise in complex symptom management, advance care planning, and interdisciplinary collaboration
  • Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions
