About the role

Career opportunity in Palliative Care at AccentCare. This position is not remote and requires coverage in Loveland and surrounding areas.

Responsibilities

  • Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
  • Perform patient assessment, history and physical; recommendations on aggressive symptom-management.
  • Maintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basis.
  • Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family.
  • Provide support at time of death.
  • Prescribe medication as permitted by the state and/or the State Board of Nursing.
  • Participate in continuum of care for patients that are hospice appropriate and ongoing education programs.

Requirements

  • Graduated from an approved school of professional nursing.
  • One year experience as a Nurse Practitioner is preferred.
  • Licensed to practice as a Nurse Practitioner in the state of agency operation.
  • Possess and maintain valid CPR certification while employed in a clinical role, preferred.
  • Must be a licensed driver who can travel to all business locations.
  • Meet the regulations and requirements of the state(s) in which program provides services.

Qualifications

  • Active DEA license in the state where care is being provided.
  • Preferred to have a valid driver's license.

  • Strong clinical judgment and decision-making skills.
  • Knowledge of palliative care principles and practices.

Pay

$50.00 - $62.00 per hour.

Schedule

Part-time (Looking for coverage during 8:00am - 5:00pm business hours, 24 hours weekly).
