Nurse Practitioner - Palliative

Key Duties and Responsibilities

• Assists with the provision of palliative care in accordance with local, state, and federal regulations.
• Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
• Implements interventions to support the patient with chronic conditions to decrease recurring hospitalizations.
• Collaborates with care team to manage referrals, follow up calls, and coordination of care.
• Facilitates the patient’s transition within and between health care settings, e.g., admitting, transferring, and discharging patients.
• Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
• Makes age-appropriate history and physical for complex acute, critical, and chronically ill patients.
• Initial home evaluation and monthly visits.
• Attends IDT, and other team meetings as required.

Education and Experience

• Will consider less NP experience with RN hospice and/or homecare experience.
• Minimum of 5 years clinical hospice and palliative care experience.
• 2 years’ experience in the Nurse Practitioner role preferred.
• Must work well independently and within a team.
• Strong computer skills, including Word, Excel, and PowerPoint.
• At least 2 years NP experience with hospice and/or palliative care experience (either NP or RN).
• Current Advanced Practice Registered Nurse (APRN) Licensure.
• Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful.
• Nurse Practitioner certification in Hospice or Palliative Care within 1 year of starting the position.
• Graduate of an accredited School of Nursing.
• Knowledge of clinical standards of care.
• Education and/or Experience: Completion of Nurse Practitioner accredited program.
• Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems.
• Minimum of two (2) years clinical experience, desired.
• Exceptional interpersonal skills & superb communication a must.
• At least one (1) year of experience in Hospice or Palliative Care.

• Excellent judgment in handling confidential and sensitive information.

• Ability to work across locations and time zones.

Licenses/Certifications

• Must be Board certified.
• Current Advanced Practice Registered Nurse (APRN) Licensure.
