Key Duties and Responsibilities
- Assists with the provision of palliative care in accordance with local, state, and federal regulations.
- Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
- Implements interventions to support the patient with chronic conditions to decrease recurring hospitalizations.
- Collaborates with care team to manage referrals, follow up calls, and coordination of care.
- Facilitates the patient’s transition within and between health care settings, e.g., admitting, transferring, and discharging patients.
- Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
- Makes age-appropriate history and physical for complex acute, critical, and chronically ill patients.
- Initial home evaluation and monthly visits.
- Attends IDT, and other team meetings as required.
Education and Experience
- Will consider less NP experience with RN hospice and/or homecare experience.
- Minimum of 5 years clinical hospice and palliative care experience.
- 2 years’ experience in the Nurse Practitioner role preferred.
- Must work well independently and within a team.
- Strong computer skills, including Word, Excel, and PowerPoint.
- At least 2 years NP experience with hospice and/or palliative care experience (either NP or RN).
- Current Advanced Practice Registered Nurse (APRN) Licensure.
- Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful.
- Nurse Practitioner certification in Hospice or Palliative Care within 1 year of starting the position.
- Graduate of an accredited School of Nursing.
- Knowledge of clinical standards of care.
- Education and/or Experience: Completion of Nurse Practitioner accredited program.
- Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems.
- Minimum of two (2) years clinical experience, desired.
- Exceptional interpersonal skills & superb communication a must.
- At least one (1) year of experience in Hospice or Palliative Care.

- Working knowledge of company policies, procedures, and operations.
- Excellent composition, grammar, and business language skills.
- Excellent communication and interpersonal skills with the ability to effectively communicate with all levels of management, patients, and family members.
- Creative, flexible, well organized, resourceful, and detail-oriented.
- Excellent judgment in handling confidential and sensitive information.
- Ability to work independently, set priorities and handle multiple tasks with a high level of efficiency.
- Establishing and maintaining cooperative working relationships with others.
- Ability to work across locations and time zones.
Licenses/Certifications
- Must be Board certified.
- Current Advanced Practice Registered Nurse (APRN) Licensure.
