We are seeking a compassionate and motivated Nurse Practitioner (NP/CRNP) or Physician Assistant (PA) to join our growing practice in Williamsport, PA. Our providers deliver comprehensive medical care to residents in Skilled Nursing, Assisted Living, and Independent Living communities. As a Nurse Practitioner or Physician Assistant, you will provide primary and chronic care management services to residents in long-term care communities. Responsibilities

  • Provide primary and chronic care management services to residents in long-term care communities.

    • Daily scheduling and care coordination
    • Documentation and note management
    • Facility communication
    • Prescription refills and medication requests
    • Forms, orders, and administrative tasks
    • Communication with nursing staff, families, pharmacies, and other healthcare partners

Pay

Compensation up to $125,000 annually with a total compensation package of $133,000–$150,000+ annually.

Qualifications

  • Current or eligible Pennsylvania Nurse Practitioner or Physician Assistant license
  • Current DEA registration or ability to obtain
  • Experience in geriatrics, long-term care, skilled nursing, family medicine, internal medicine, or primary care is preferred but not required
