Our providers deliver comprehensive medical care to residents in Skilled Nursing, Assisted Living, and Independent Living communities. As a Nurse Practitioner or Physician Assistant, you will provide primary and chronic care management services to residents in long-term care communities. Responsibilities
- Provide primary and chronic care management services to residents in long-term care communities.

Pay
Compensation up to $125,000 annually with a total compensation package of $133,000–$150,000+ annually.

Qualifications
- Current or eligible Pennsylvania Nurse Practitioner or Physician Assistant license
- Current DEA registration or ability to obtain
- Experience in geriatrics, long-term care, skilled nursing, family medicine, internal medicine, or primary care is preferred but not required
