About the role
The Hospital of Central Connecticut is seeking a Nurse Practitioner or Physician Assistant to join their Palliative Care team. The Advanced Practitioner (APP) will work in collaboration with the attending physician and members of the interdisciplinary care team to deliver expert palliative care services to patients and their families living with chronic serious or acute life threatening illness, or those nearing end of life.
Responsibilities
- Work in collaboration and/or supervision with the practice physician(s)
- Perform routine physical examinations
- Formulate medical, educational, and psychosocial plans to treat patients
- In consultation with the physician(s) and care team members, develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists
- Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identify problems outside of their scope of practice and refer appropriately to physician
Qualifications
- Licensure: Unrestricted State of Connecticut Medical License (or eligible)
- Training: Physician Assistant or Nurse Practitioner
- Preferred Experience: 1+ years’ experience preferred
- National Board Certification in Palliative Care
