Nurse Practitioner or Physician Assistant - Advanced Care at Home Program

About the role

The Advanced Care at Home (ACH) program within the Division of Hospital Internal Medicine at Mayo Clinic Arizona is an innovative care model delivering comprehensive, hospital-level care to patients in their homes. The Nurse Practitioner/Physician Assistant (NP PA) plays a crucial role in this transformation by providing safe, high-quality, patient-centered care.

Responsibilities

• Independently and collaboratively manage patients receiving hospital-level care in the home setting.
• Obtain comprehensive patient histories and perform physical examinations.
• Formulate diagnostic impressions and develop evidence-based treatment plans.
• Order, interpret, and perform diagnostic and therapeutic procedures within scope of practice.
• Maintain monitoring of patient progress and evaluate the effectiveness of therapeutic interventions.
• Coordinate consultations, referrals, and transitions of care.
• Provide education, counseling, and support to patients and their families.
• Collaborate closely with interdisciplinary care teams to ensure seamless, high-quality care delivery.
• Maintain prescribing authority and clinical privileges as required by Arizona regulatory and institutional standards.
• Serve as an advocate for quality, safety, service excellence, and continuous improvement within the ACH program.
• Promote awareness and understanding of the ACH model among patients, families, and healthcare professionals.

Qualifications

• Acute Care Nurse Practitioner: Master’s-prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current AZ RN and APRN license and certification as an ACNP.
• Physician Assistant: Graduate of an accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the AZ State Board of Medical Practice.
• At least one year of hospital-based experience.
• Flexible to work 12-hour shifts, weekends, and holidays.
• Experience providing care in patient’s homes or via telehealth/remote monitoring is preferred.
• Active BLS and ACLS certification from American Heart Association or American Red Cross at start date.
• Evidence of application for DEA certificate must be provided upon hire and obtained within 90 days of hire.
• A valid driver’s license and an acceptable driving record per an annual motor vehicle record check are required.
• Excellent verbal and written communication skills and strong computer skills.
