Role Overview

This position involves providing in-home clinical care to patients in Buchanan and Andrew counties, with daily travel required. The nurse practitioner conducts comprehensive assessments, manages acute and chronic conditions, and collaborates with an interdisciplinary team to deliver holistic care.

Responsibilities

• Conduct in-home health assessments and screenings
• Manage health conditions and acute episodes
• Collaborate with healthcare team and document care accurately
• Engage in advanced care planning and patient counseling

Qualifications & Skills

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure in Missouri and valid driver's license with reliable transportation
• Ability to navigate varied environments and perform clinical duties
• PREFERRED: 1+ years of clinical experience in a home health, geriatric, or long-term care setting; familiarity with electronic medical records and complex patient needs
