Nurse Practitioner, Optum Care at Home - Buchanan, Andrew Counties, MO

About the Role

This position involves providing in-home care to high-risk patients across Buchanan and Andrew counties, focusing on holistic health management. The role requires travel to patient homes, conducting comprehensive assessments, managing acute and chronic conditions, and facilitating care planning. It emphasizes collaboration within an interdisciplinary team, documentation accuracy, patient education, and support for advanced illness conversations.

Responsibilities

• Perform in-home assessments including medical history, vital signs, and screenings
• Manage acute episodes and chronic condition exacerbations with diagnostics and interventions
• Document thoroughly with ICD-10 coding
• Collaborate with the care team and provide patient and caregiver education
• Engage in advanced care planning discussions

Requirements

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure in Missouri or ability to obtain it
• Valid driver’s license and reliable transportation
• Ability to navigate varied environments and perform physical tasks as needed

Preferred Qualifications

• Clinical experience in geriatrics, home health, or long-term care
• Familiarity with complex needs and end-of-life discussions
• Proficiency with electronic medical records
