Responsibilities
- Manage a panel of patients

- Collaborate with a dedicated support team, including onsite lab and pharmacy resources
Qualifications
- Unrestricted Arizona NP or Physician Assistant license
- National certification through AANC, AANP, or NCCPAC
- CURRENT DEA registration and BLS certification
Preferred Skills
- Experience with Value Based Care Models and outpatient primary care
- Knowledge of HEDIS, CAHPS, HCC coding, and Medicare Wellness visits
- Proficiency with EMR systems, especially EPIC
