Nurse Practitioner (NP) / Physician Assistant (PA)

Medical Director Services PC is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide primary care services at a skilled nursing facility in Torrington, CT.

Medical Director Services was founded in 2016 during a shift in skilled nursing facilities toward value-based care and higher-acuity residents. We prioritize quality, customer service, and treating residents in place to reduce unnecessary hospitalizations.

Responsibilities

• Serve as the primary care provider for long- and short-term residents.
• Assess patient health through visits, physical examinations, and medical history reviews.
• Collaborate with attending physicians and nursing home staff in a team-based care model to deliver the highest standard of care.
• Build relationships with residents and families, educating them and formulating evidence-based care plans aligned with patient needs and preferences.
• Comply with federal, state, and local legal and professional requirements, including staying updated on legislation and advising management on necessary actions.
• Maintain professional knowledge through educational workshops, publications, networking, and participation in professional societies.
• Ensure a safe and clean working environment by adhering to procedures, rules, and regulations, while respecting cultural, personal, and religious beliefs.
• Review and ensure timely, accurate charting, documentation, and administration of care.

Requirements

• Board certification as a Nurse Practitioner or Physician Assistant.
• Proficiency in health promotion and maintenance, clinical skills, patient education, medical teamwork, bedside manner, infection control, medication administration, pain management, and self-development.
• Thoroughness and attention to detail.

Extensive training will be provided.

Pay

Excellent salary commensurate with experience.
