About the role

Responsibilities

  • Serve as a proactive, collaborative team member in a hospice care environment.
  • Conduct comprehensive adult/geriatric assessments of hospice patients and evaluate and manage the plan of care.
  • Demonstrate a commitment to caring for patients and partners.
  • Complete all documentation and records in a timely and accurate manner.
  • Follow established policies and procedures as related to the scope of practice.
  • Maintain continuity of patient care in a hospice setting.

Requirements

  • Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in the state of practice.
  • Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.
