About the role

As a Nurse Practitioner in our Hospice and Palliative Care Services, you will deliver compassionate, patient-centered care to individuals with life-limiting illnesses. This role involves collaborating with a multidisciplinary team to ensure comfort, dignity, and quality of life for patients and their families.

Responsibilities

  • Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs.
  • Develop and implement individualized care plans in collaboration with the hospice team.
  • Provide symptom management, pain control, and palliative care interventions.
  • Educate patients, families, and caregivers on hospice care, disease progression, and end-of-life planning.
  • Serve as a resource for hospice nurses, aides, and other team members.
  • Participate in interdisciplinary team meetings to review and adjust care plans.
  • Ensure compliance with federal, state, and organizational regulations for hospice care.
  • Provide on-call support as needed for urgent patient needs.

Requirements

  • Current, unrestricted Nurse Practitioner license in the state of practice.
  • Certification in Hospice and Palliative Care (preferred but not required).
  • Minimum of 2 years of clinical experience as a Nurse Practitioner, with hospice or palliative care experience strongly preferred.
  • Strong clinical skills in pain and symptom management.
  • Excellent communication, empathy, and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Reliable transportation for travel to patient homes and care facilities.
