Nurse Practitioner (NP)
Scope Of Position
The Nurse Practitioner is responsible for providing general medical care and treatment to residents at Rockingham County Rehabilitation and Nursing Center under the direction of the Physician. This role operates within the scope of practice for Nurse Practitioners and in accordance with State and Federal regulations, as well as established standards of care. The Nurse Practitioner supports the delivery of safe, effective, and individualized resident care.

The Nurse Practitioner plays a key role in this mission by assessing residents’ health status, developing and managing plans of care, and evaluating outcomes to ensure appropriate treatment and quality care.
- Orders and interprets diagnostic and therapeutic tests
- Prescribes pharmacologic and non-pharmacologic treatments
- Maintains accurate billing and documentation
- Completes discharge summaries and prescriptions
- Manages prior authorizations, renewals, and medications
- Communicates effectively with residents, families, and the healthcare team
- Provides education on care plans, treatments, medications, and disease processes
- Manages complex wound care
- Provides mobility assistance
- Collaborates on medical procedures
- Delivers end-of-life care
- Participates in Quality Improvement/QAPI initiatives
- Supports a resident-centered care environment
- Ensures compliance with all regulatory and safety standards, including reporting requirements
Required Education And Experience
- Graduate of an accredited School of Nursing (RN)
- Graduate of an accredited Nurse Practitioner Program
- Or an equivalent combination of education, work, and experience will be considered
- Current Advance Practice Registered Nurse (APRN) License
- National Board Certification
- Current licensure and credentials required by the State of NH to practice as Nurse Practitioner
- CPR / Basic Life Support (BLS) certified
