Nurse Practitioner (NP)
The Parker Jewish Institute for Healthcare and Rehabilitation, located in New Hyde Park, NY, is seeking a Nurse Practitioner to join our team in the Medicine Department.
About the role
In this role, the Nurse Practitioner will report to the Medical Director or the supervisory Attending Physician and is responsible for delivering compassionate and quality medical care to assigned residents in coordination with other members of the healthcare team. Shifts available: 11:00 AM–11:00 PM or 3:00 PM–11:00 PM.
Responsibilities
- Examine residents and deliver quality care in varied settings including subacute, long-term, rehab, and outpatient care
- Assist in resident/patient/family education
- Re-examine, re-evaluate, and modify care plans as necessary, including maintaining proper documentation in resident charts
- Review all diagnostic testing results and make recommendations based on findings
- Educate and supervise rotating medical and physician assistant students

Requirements
- Graduate of an accredited nurse practitioner program
- Active license as an NP in New York State, with prescriptive privileges
- NYS Board Certified
- Proficient in SigmaCare or another EMR (Electronic Medical Record) software
- Prior experience with the geriatric population preferred
- Two or more years of experience in a sub-acute or long-term care facility
