Our Nurse Practitioner (NP) providers become an integrated part of each community—partnering with residents, families, nursing staff, therapists, and collaborating physicians to reduce avoidable hospitalizations while improving quality outcomes.

Responsibilities

• Provide comprehensive primary care to residents in skilled nursing, long-term care, and assisted living settings as the primary on-site Nurse Practitioner (NP) provider for your assigned facility.

Requirements

• Master's degree or higher from an accredited Nurse Practitioner program
• National board certification (FNP, AGNP, AGPCNP, or GNP)
• Active Missouri APRN and RN licenses
• Eligible to practice under a Missouri Collaborative Practice Arrangement
• DEA and Missouri BNDD registration (or ability to obtain)
• Medicare (PECOS) and Medicaid enrollment eligibility
• Current BLS Certification
• Valid driver's license, reliable transportation, and current auto insurance
• Experience using Electronic Health Records (EHR)

Preferred Qualifications

• Two (2) or more years of Nurse Practitioner (NP) experience
• Experience in Family Practice, Adult, Skilled Nursing, Long-Term Care, Geriatrics, Home-Based Primary Care, or Acute Care
• Experience with PointClickCare or other Long-Term Care EHR systems
• Knowledge of CMS Skilled Nursing Facility regulations
• Familiarity with value-based care models, ACOs, I-SNPs, and geriatric prescribing principles

Pay

Competitive Total Compensation: $125,000-$150,000
