About the role
The Evergreens is seeking a Nurse Practitioner to provide services across the continuum of care (independent living, assisted living, and skilled nursing). This role involves building and managing an outpatient Primary Care Practice on campus, as well as providing care for our assisted living and skilled nursing residents.
Responsibilities
- Plan and direct all functions of the resident care clinical area, including management of team members and the day-to-day operations of the practice.
- Manage resident healthcare needs in compliance with federal and state regulations.
- Maintain knowledge of the long-term care industry and geriatrics through continuing education.
- Serve as clinical preceptor for Nurse Practitioner students.
- Play a key role in the development and implementation of preventative and well-being services, providing proactive care to assist residents to thrive and age in place.
Requirements
- Master’s degree in Advanced Nursing; preferably in geriatrics.
- Must be a licensed Nurse Practitioner with prescriptive authority in the applicable work state.
- Minimum of five years' experience working as a Nurse Practitioner in a primary care, urgent care, or other medical office setting, particularly within geriatrics.
- Experience in continuing care, skilled nursing care, long-term care, and/or in an assisted living environment.

Pay
$105,029.00 - $129,237.00 / year. Starting rate will vary based on skills and experience.
Schedule
Nightly shifts, rotating weekends.
