Nurse Practitioner (NP)

About the role

We are seeking nurse practitioners committed to providing quality, compassionate care to seriously ill patients. You will collaborate with a team to deliver comprehensive palliative and hospice services.

Responsibilities

• Care for patients to alleviate burdensome symptoms and align medical treatments with patients' goals and values
• Collaborate with a team of clinicians and teammates to provide the best care possible
• Provide ongoing palliative care and hospice service
• Participate in education initiatives for the medical community in your territory

Qualifications

• Graduate of an accredited nursing program
• 2 years of experience within the last 5 years in chronic disease management/long-term care, with hospice background preferred
• Reliable transportation
• Ability to sit, stand, bend, move intermittently, and lift up to 100 lbs effectively
