Nurse Practitioner (NP)

About the role

We are seeking nurse practitioners committed to providing quality, compassionate care to seriously ill patients. Your role includes caring for patients to alleviate burdensome symptoms, collaborating with a multidisciplinary team, and delivering both palliative and hospice services.

Responsibilities

• Care for seriously ill patients to alleviate burdensome symptoms and align medical treatments with patients' goals and values.
• Collaborate with a team of like-minded clinicians and teammates to deliver the best care possible for patients.
• Provide ongoing palliative care and hospice service in your territory.
• Engage in education related to hospice and palliative care.

Requirements

• Graduate of an accredited nursing program.
• At least 2 years of experience within the last 5 years in chronic disease management or long-term care, with a preference for hospice background.
• Reliable transportation.
• Ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs and bear the weight of an average adult effectively.
