Work sites: 2 days a week at Apple Valley Care Center (Apple Valley, CA 92308) + 3 days a week at Upland Care Center (Upland, CA 91786).

Schedule

5 days a week, Monday – Friday: 8:00 am - 5:00 pm.

Responsibilities

  • Provide health care for patients in collaboration with a physician through advanced assessment and identification of health states, including abnormal conditions, diagnosing, treatment therapies, and evaluation.
  • Perform care in the specialty area of nephrology and for patients on dialysis.
  • Utilize evidence-based clinical practice standards to provide direct clinical services within prescribed guidelines and physician instructions.
  • Act independently to perform physical assessments, diagnostic functions, specialized risk assessments, and make critical decisions regarding a wide variety of health situations, including high risk and crisis situations.
  • Acquire patient health histories and evaluate patients, performing physical examinations.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and health care needs.
  • Discuss cases with physicians and formulate and document care plans.
  • Prescribe medication, therapies, and other forms of treatment.
  • Evaluate the patient’s response to therapy.

Requirements

  • Master of Science in Nursing.
  • Minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or equivalent combination of education and experience.
  • Active Nurse Practitioner Board Certification.
  • Current, unrestricted RN license and NP license in the state of California.
  • Certification in a specialty or minimum of 1 year of experience in the specialty area of nephrology.

Preferred Skills

  • Bilingual communication skills in English and Spanish.
  • Experience in either Primary Care or an Internal Medicine setting with adults and geriatric patient populations.

Pay

Compensation range: $119,313.00 to $178,968.00 (national market average; roles in premium or high-cost geographic markets may fall above this range). This position is bonus eligible based on individual and company performance.
