About the role

The Nurse Practitioner role at Greater Good Health involves leading patient care at our primary care clinics. The role requires managing a patient panel, working closely with the care team, and delivering exceptional patient experiences.

Responsibilities

  • Build a strong culture & foster community.
  • Welcomes all and cares empathetically for patients and team members.
  • L-listens and observes the needs of each patient to ensure they are supported throughout their visit to provide an excellent patient experience.
  • Flexes to fit the needs of the patient and care team as needed.
  • Delegates to Medical Assistants/LPN's to support delivery of procedures, telephonic support and patient instructions.
  • Fosters a culture of engagement and demonstrates Greater Good Health core values.
  • Delivers exceptional clinical care to patients.
  • Provides full scope of primary care services including well acute, and chronic condition management.
  • Takes medical histories, examines patients, makes diagnoses, and prescribes medication to the extent allowable by state guidelines.
  • Performs Annual Wellness Visits and Health Risk Assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific clinical coding and documentation.
  • Flexibility to conduct visits through various modalities including in-clinic, telehealth and potentially home visits as needed.
  • Manages patient panels and "quarterbacks" patient care with patient’s team to deliver high-touch, high-quality care.
  • Orders diagnostic ss and interprets results.
  • Performs appropriate screenings, ss, and in-office procedures.
  • Provides patients with education on preventive measures, lifestyle management, and disease management.
  • Makes referrals to other providers, specialists, sing facilities, agencies, hospitals, or ancillary providers.
  • Develops and manages relationships with specialty consultants; monitors follow up after patient's visit.
  • Makes post-hospitalization follow up visits.
  • Assists with telephonic triage and outreach as needed.
  • Provides after-hours coverage on a rotation with other Greater Good Health clinicians.
  • Performs prior authorizations and peer-to-peer reviews as needed.
  • Drive clinical quality and safety to ensure high-quality patient care.
  • Responsible for accurate HCC coding to ensure comprehensive capture of patient’s health conditions.
  • Responsible for meeting key quality and clinical outcome measures.
  • Promotes continuous improvement and nurtures growth.
  • Responsible for accurate, complete, timely charting in the Electronic Health Record (EHR).
  • Leads and participates in care team meetings to discuss patient care and clinic operations.
  • Helps onboard new NPs to Greater Good Health and precept NP students, if applicable.
  • Participates in regular chart audits and trainings.
  • Cross-functional collaboration with all team members to complete ad-hoc projects.
  • Supports and participates in Greater Good Health-led or co-sponsored events by providing education at in-clinic and community events.
  • Collaborates with leadership on ad hoc projects or with business development initiatives.
  • Attends and participates in regular staff operational and educational meetings.
  • Promote And Support a Premier Patient And Teammate Experience.
  • Promotes high-quality patient care and outstanding patient experience as reflected in patient satisfaction scores.
  • Collaborates and expertly communicates with all team members so that patient needs are met throughout their visit and when they are outside the clinic to assure a smooth patient flow/experience.

Qualifications

  • Active FNP or AGNP license in the state of Montana.
  • CPR/BLS Certification.
  • Prescriptive Authority by the MT state board of Nursing or Medical Examiners and DEA License.
  • Practice in Primary Care and independent patient care.
  • 3-5+ years of experience.
  • Valid Montana Driver’s License.
  • Proficiency with Microsoft Word, Excel, and PowerPoint.
  • Adult Gerontology specialty board certification (Preferred).
  • Value-based panel management and care coordination experience (Preferred).
  • New practice implementation and growth experience (Preferred).
  • Electronic Health Record (EHR) experience (Preferred).
  • Experience delivering educational sessions (Preferred).
