Salary: $110,000 - $175,000 + mileage & full benefits. Relocation available!

Responsibilities

• Deliver field-based care by conducting comprehensive primary care visits and physical exams in patients’ homes.
• Manage chronic disease by creating longitudinal care plans for high-risk patients.
• Collaborate across the continuum with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER visits by treating acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Requirements

• Master’s Degree (Nurse Practitioner) with a preferred focus on Adult, Geriatric, and/or Family.
• Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Valid driver’s license and reliable vehicle for local field-based travel.
