Job Description Summary
Collaborating with the healthcare team, the Nurse Practitioner - Palliative Care leads the development and implementation of an interdisciplinary care plan for patients with complex health needs. They also facilitate coordinated, patient-centered care and improve patient health outcomes and quality of life through excellent symptom management and coordination with community services.
Job Requirements, Prerequisites And Essential Functions
Pay Range: $75.06 - $87.56 - $102.64
Assessments: Conducts initial in-person and/or virtual assessments for patients, assessing goals, spiritual/religious beliefs, relationships, social services, substance use, and behavioral health.
Patient Care Plan: Develops and implements a patient-centered, individualized care plan based on personal health goals, supporting patients in goal identification, risk assessment, and care team communication.
Patient Education: Educates patients and families on care plans, medication management, advanced illness planning, and health care system navigation.
Interaction / Influence (Internal): Manages patient care with interdisciplinary teams, representing the program internally and externally, participating in meetings, and contributing to team decisions.
Training: Attends and participates in staff development trainings and team meetings, sharing successes and challenges.
Safety: Identifies and reports quality and risk management concerns, adheres to policies, and maintains accurate patient records.
Strategy / Improvements: Contributes to developing a care delivery system sensitive to individual patient needs, promoting effective resource utilization, and coordinating care across the continuum.
Qualifications
Education: Graduate of accredited school of nursing, BSN preferred.
Licenses/Certifications: Current Registered Nurse license, Nurse Practitioner license, DEA license, and national board certification.
Experience: Three (3) or more years of experience in acute patient care or community-based home health/hospice care.
Skills: Palliative Care experience preferred, motivational interviewing knowledge, experience with Medi-Cal regulations, bilingual language skills, broad clinical background.
